• Medicare, Veterans Affairs or Health Care Cards
• Health Insurance Information
• Copies of any previous medical and/or surgery records
• Copies of imaging/scans related to your appointment

Cancellations/Changes to appointments:

If for some reason you are unable to attend your appointment, please phone our office at your earliest convenience to cancel or amend your appointment. Cancellations/changes must be made at least 1 day prior to your allocated appointment. Multiple cancellations may result in failure to book further appointments at the practice’s discretion.
